Health Clinic Social Worker / Case Manager provides direct client
support, case management, and care coordination to individuals
receiving mental and behavioral health services. This role ensures
clients have access to appropriate treatment, resources, and
support systems, and partners closely with clinicians and community
providers to promote holistic, client-centered care. The Social
Worker / Case Manager maintains a caseload, conducts assessments,
tracks progress, and ensures compliance with documentation
standards and ethical practice guidelines. Key Responsibilities
Client Support & Case Management - Develop individualized service
plans. - Provide case management, including referrals, advocacy,
and linkage to internal and external services. - Coordinate care
with multidisciplinary providers to ensure continuity and quality
of services. - Support clients in navigating housing, healthcare,
benefits, employment, and community resources. - Monitor client
progress, update service plans, and maintain consistent follow-up.
Clinical & Therapeutic Support - Provide supportive counseling and
crisis intervention within scope of role. - Participate in
treatment planning with clinicians, care teams, and outside
partners. - Collaborate with therapists, psychiatrists, and medical
staff to support coordinated care. - Conduct outreach and
engagement for clients with high needs or inconsistent
participation. Documentation & Compliance - Maintain accurate,
timely documentation in accordance with agency, county, and state
requirements. - Complete assessments, progress notes, service
plans, and required reports. - Ensure adherence to confidentiality,
HIPAA, and ethical guidelines. - Participate in audits, chart
reviews, and quality improvement processes. Teamwork &
Collaboration - Attend team meetings, case conferences, and
supervision sessions. - Work collaboratively with internal teams to
support trauma-informed care. - Engage with community agencies,
shelters, medical providers, and public programs. Program Support -
Assist with client intake, engagement, and orientation activities.
- Participate in program development, outreach, and community
education efforts. - Support grant reporting and evaluation tasks
as needed. Requirements: Required - Bachelor's degree in Social
Work, Psychology, Counseling, Human Services, or a related field. -
Experience working with individuals facing mental health, substance
use, or psychosocial challenges. - Strong knowledge of community
resources, crisis intervention, and trauma-informed practices. -
Excellent communication, documentation, organizational, and
interpersonal skills. - Ability to work effectively in a
fast-paced, client-centered clinical environment. Preferred -
Master's degree in Social Work, Psychology, Counseling, or a
related field. - Experience in a community mental health,
outpatient clinic, or nonprofit setting. - Bilingual language
skills. - Familiarity with electronic health record (EHR) systems.
Additional Information: Work Environment - Primarily onsite in a
clinic setting; may involve fieldwork or home/community visits. -
Requires the ability to manage multiple priorities and maintain
clear professional boundaries. - May require occasional evening or
weekend hours depending on program needs. Physical Requirements -
Ability to sit, stand, walk, and climb stairs intermittently. -
Ability to conduct outreach in community settings. - Reasonable
