Summary: We are seeking an experienced and compassionate Admission
Manager to lead our hospice admission team. The successful
candidate will be responsible for managing the admission process,
ensuring timely and accurate intake of patients, and providing
exceptional customer service to patients, families, and referral
sources. Key Responsibilities: 1. Manage the admission process,
including initial screenings, assessments, and coordination with
interdisciplinary teams and intake 2. Develop and maintain
relationships with referral sources, including physicians,
hospitals, and other healthcare providers. 3. Ensure compliance
with regulatory requirements, including Medicare and Medicaid
guidelines. 4. Collaborate with clinical teams to ensure seamless
transitions and high-quality patient care. 5. Analyze admission
data and trends to identify opportunities for improvement. 6.
Develop and implement strategies to increase admissions and grow
the hospice program. 7. Provide leadership and guidance to
admission staff, including training, mentoring, and performance
management. 8. Foster a culture of compassion, empathy, and respect
within the admission team. 9. Participates n theon call rotation
10. Provide education/inservice to internal and external customers
about hospice philosophy and admission criteria 11. Do ridealongs
with Sales representatives 12. Shadow admission nurses on a regular
basis 13.Visit facilities to do followup on how admission met and
improve admission process as needed Requirements: Bachelor's degree
in a related field (e.g., nursing, social work, business
administration) Experience in hospice or healthcare admissions,
preferably in a leadership role Strong communication and
interpersonal skills Ability to analyze data and make informed
decisions Knowledge of regulatory requirements and compliance
standards Compassionate and empathetic approach to working with
patients and families Preferred Qualifications: At least one year
Experience as an admission manager or Admission nurse Hospice or
palliative care experience Leadership or management experience
Certification in hospice or palliative care (e.g.CHPN) Experience
with electronic medical records (EMRs) and admission software
Physical Requirements: Ability to sit and stand for extended
periods Frequent movement between offices, departments, or external
meetings Occasional travel may be required Ability to lift, carry,
or move light objects (e.g., files, papers) Visual acuity to read
and use computer screens Hearing ability to communicate effectively
with staff, patients, and external partners Ability to type and use
computer keyboard Specific Physical Demands: Sitting: Up to 60% of
workday Standing: Up to 20% of workday Walking: Up to 20% of
workday Lifting: Up to 10 pounds occasionally Bending, reaching, or
